✨ Our Journey at Plan Wise Living ✨

Plan Wise Living established from lived experience. In 2017, our founder, Giuliana, started navigating the NDIS for her daughter Gabriella, who lives with multiple disabilities and complex health needs. Through that journey, she saw firsthand how important collaboration, clear communication, and the right supports are in achieving meaningful outcomes.

What started as a personal mission grew into a disability support provider, where we proudly supported participants and families for five years (2017-2022). In 2022, we transitioned into a dedicated Support Coordination company so we could focus on helping participants navigate the NDIS with clarity and confidence.

For the past four years, we’ve been providing Support Coordination and Specialist Support Coordination, assisting participants with:
✅ Understanding NDIS plans and budgets
✅ Connecting with the right providers and services
✅ Building sustainable supports
✅ Ongoing guidance, advocacy and future planning

At Plan Wise Living, we understand the system — because we’ve lived it. Each of our support coordinators bring a wealth of both lived and professional experience within the disability sector, giving us a true understanding of the challenges and opportunities you face. This combination of personal insight and hands-on work fuels our passion for guiding participants through their NDIS journey, connecting them with the right supports, and advocating with empathy, knowledge, and genuine care.
