On Women’s Day, we celebrate the extraordinary roles women play in shaping our families, communities, and nation. Women are the carers, educators, leaders, and innovators who hold society together, balancing responsibilities with strength and determination.

Elderly women, in particular, stand as enduring pillars of care. In South Africa, they make up the majority of older persons and many head households, often relying on social grants, while continuing to nurture and support extended families. Despite facing challenges such as widowhood, financial strain, and health concerns, they remain a source of guidance and compassion.

The OPCA-WC honours all women, and especially our elderly women, whose dedication continues to inspire generations.

According to the South African Human Rights Commission, over 70% of South Africa’s older population relies on an under-resourced public healthcare system. The Older Persons Act (2006) promises accessible clinics and sufficient and reliable services, but without enforcement, these rights remain aspirational.

With a severe shortage of specialised geriatric services, the burden of long-term care falls heavily on households without adequate state support. To build a truly equitable society, we must strengthen community-based infrastructure, expand healthcare capacity, and support the communities caring for our elders.

Across South Africa, thousands of care workers support older persons every day, yet most remain undertrained and invisible in the system.

Without accredited qualifications, they cannot be formally recognised. Without recognition, they are denied support, adequate preparation and professional standards. Without stronger systems, elders are left exposed to unsafe and uneven care.

It’s time to invest in care worker registration and accreditation and build a system where both carers and elders are protected and respected.

Mr. Dumisani Harold Jonas, 65, suffered a stroke and was left living alone in a shack, neglected, unkempt, and without proper care. His home doubled as a bathroom, was infested with lice, and reeked of neglect. When the nursing team sought help from the local clinic, none was available.

This is not just one man’s struggle. It is a stark reminder of the inequality in ageing in South Africa; elders who contributed to society are too often abandoned, with rights promised in law but denied in practice. Access to dignified housing, health care, and essential products remains uneven, leaving the poorest elders most vulnerable.

Ikamva Labantu's intervention of cleaning his shack, restoring his dignity, and securing bedding, food, and medical referrals shows what care should look like. But it also exposes the gap: why must NGOs step in where the state and health system fail?

Ageing should never mean neglect. It should mean dignity, safety, and care. Until elder rights are fully realised, inequality in ageing will remain one of South Africa’s deepest injustices.

Let’s demand stronger accountability, accessible elder care services, and investment in community health systems. Because no elder should be forgotten.
