Trustbridge Trustbridge is a community nonprofit that provides 24/7 support for families facing serious illness.

From home health, to hospice, and caregiver support, TrustBridge offers assistance for families facing any stage of illness, twenty-four hours a day. TrustBridge combines Hospice of Palm Beach County and Broward County and Hospice by the Sea, Focus Care Home Health, and Harbor Palliative Care. Since 1978, our expert staff has cared for nearly 200,000 patients nearing end of life. As a non-profit,

501(c)(3) organization, we care for all who want and qualify for our services, regardless of ability to pay. We offer the physical, emotional and spiritual care that brings comfort and dignity to those with terminal illness and their loved ones. Our multi-cultural staff respects all religions and cultural traditions. For families who need emotional support, we offer grief support services including group counseling. Meet Hannah, an exceptional teen whose kindness is making a meaningful impact. Through her fundraising efforts, she helped create more than 200 Shabbat boxes for patients and their families at our Care Centers, bringing comfort, tradition, and connection to those who may not be able to celebrate at home.

In life’s most tender moments, the bonds we share with loved ones is everything. As we close out Patient Experience Week, we are honored to share Brittany’s heartfelt story about her beloved Aunt Bee.

Brittany’s reflection on her aunt’s final chapter is a powerful reminder of what compassionate hospice care can mean—not only for patients, but for the loved ones walking beside them.
