Hearts to Homes Hearts to Homes provides household furnishings and furniture to youth aging out of foster care.

Originally created in March 2016 as a program of a single foster care agency, Hearts to Homes is now an independent organization helping more young adults from more foster care agencies in New York City, Westchester, Nassau and Suffolk counties. Our mission is to assist newly independent young adults who have just aged out of foster care by providing the essentials to furnish a new home.

Having a kitchen is only half the battle. Having food is where true security begins.

This , we are highlighting how food insecurity is a critical issue our recipients have to deal with when transitioning out of foster care and into adulthood.

With 25% of young people living in households facing food insecurity, the need is urgent, especially for our recipients in the Bronx, which has the highest rate of food insecurity in the city.

That is why food benefit programs like SNAP and Hearts to Homes' pantry grant are so vital for our youth. We provide each recipient with $400 in grocery gift cards to help stock their kitchen with pantry essentials so they can make their own nourishing meals.

Our mission is to make sure our recipients won't go hungry when embarking on their new chapter of independence.

Thanksgiving is about more than food, it’s about knowing you have a seat at the table and a community that cares.

For young adults who have aged out of foster care, holidays can feel quiet or overwhelming.

Through our Holiday Meal Program, our volunteers assemble and deliver full grocery totes straight to our recipients' doors, bringing warmth, comfort, and care for Thanksgiving.

We are encouraged by recent data in the last 5 years from NYC Administration for Children's Services- ACS showing a five-year decline in both the number of youth in 24-hour care and those who are aging out of foster care.

In 2021, 549 young adults aged out of care. By 2025, that number fell to 358.
While fewer former foster youth are aging out, the transition into adulthood is just as challenging without support.

Those young adults still need beds to sleep in, tables to eat at, and basic essentials to turn their empty living spaces into comfortable homes. Until we are no longer needed, Hearts to Homes will be there to support former foster youth.

On , we want to highlight why setting up young adults aging out of foster care for long-term stability matters.

Nationally, between 31-46% of former foster youth experience homelessness by age 26, which is more than 7-11 times the rate of homelessness in the general population. But with housing support and resources, that cycle can be broken.

At Hearts to Homes, our recipient homelessness rate has historically averaged 12%, and our ultimate goal is 0%.

When you donate to our mission, you aren't just helping us purchase home furnishings. You are directly investing in housing security, emotional well-being, and independence for our young adults.

75 new beginnings. 75 safe spaces. 75 brighter futures.

Between June and September of this year, we are proud to have helped 75 young adults aging out of foster care transform empty living spaces into homes by providing home furnishings.

Equipping an empty apartment with all the necessary household essentials costs an average of $1,900 per home. Over the last four months alone, that was a total cost of $142,500.

We want to keep this momentum going, but we can't do it alone. It takes your generous support to help us support even more recipients.

For young adults leaving foster care, starting over shouldn't mean waiting months to furnish their new home with pieces they didn't choose.

When our recipients receive the keys to their new living space, timing and choice are important.

Marissa, our Program Manager, makes sure we tailor all home furnishings to each recipient's preferences and needs.

Our team does this work as quickly and thoughtfully as possible so our young adults can walk into a comforting home that feels authentically like theirs without delay.

The feeling of clean clothes and bedding might be something many of us don't think about.

However, for a young adult aging out of foster care without basic household supplies, going without laundry essentials can make an already overwhelming transition even harder.

With a $40 donation, you help us provide supplies, including laundry baskets, detergent, an iron, and an ironing board.

Our laundry bundle gives our recipients the tools to keep their home and wardrobe fresh as they prepare for job interviews, parenting, classes, or work.

You can help make this possible by donating here: https://heartstohomes.org/donation-bundles/

Hearts to Homes would like to thank Diane and Charlie Medici for including us in their Dahunt for the Cure sporting clays event.

On a beautiful morning in Dover Plains, NY, over 120 people gathered to compete in this fun event. We were one of six non-profits who will receive a grant from the proceeds of this event.

We appreciated that our very own Sonja Smash was invited to speak and share Hearts to Homes' critical mission with those in attendance.

Following up with our recipients is important to us. Once home furnishings are delivered to our young adults, we ask for feedback. We also ask questions like:

Was everything received?
Does everything work?
Is anything broken?

Making sure everything that is needed was received ensures a smoother transition for the young adult and values their experience.

Promises made are promises delivered.
