HOSPITAL SPOTLIGHT 🔦 This week we're featuring Backus and Windham Hospitals' Community Health Case Management Program, recipient of the 2026 Community Service Award! 🏠🥗🚌

The Community Health Case Management Program, operated jointly by Hartford HealthCare’s Backus and Windham Hospitals in the health system’s East Region, is a community-based care coordination model that goes beyond traditional hospital boundaries by embedding a dedicated interdisciplinary team directly into the community. The program makes care more accessible by building a sustainable, high-impact bridge between hospital and community.

This outreach program serves highly vulnerable populations, including individuals experiencing homelessness, seniors at risk of institutionalization, and historically underserved individuals facing complex social and medical barriers. Working in nonclinical settings, such as shelters, warming centers, senior housing complexes, soup kitchens, and senior centers, the team provides free basic health screenings (blood pressure, A1c, cholesterol via CLIA certificate), intensive case management and chronic disease support, wound care and medication reconciliation, nutrition counseling, and linkage to housing, food, transportation, and behavioral health resources.

Since the program was established in January 2023, the clinicians have facilitated more than 2,000 in-person community encounters, fostered more than 100 connections to community organizations, and submitted more than 400 referrals to healthcare services.

HOSPITAL SPOTLIGHT 🔦 This week we're featuring Yale New Haven Hospital's High-Risk Patient MRSA Prevention Initiative, recipient of the 2026 John D. Thompson Award! 👃🦠🛡️

Yale New Haven Hospital improved patient care by redesigning how high-risk patients are identified and treated to prevent hospital-onset bloodstream infections, shifting from reactive, variable practices to a standardized, proactive, risk-based nasal treatment process guided and validated by externally reported data.

Hospital-onset methicillin-resistant Staphylococcus aureus (MRSA) bloodstream infections are associated with substantial morbidity, mortality, and cost. Despite adherence to established prevention strategies — such as hand hygiene, contact precautions, and chlorhexidine bathing — data revealed persistently elevated rates, signaling the need for a fundamental change in practice.

In response, a multidisciplinary team, including infection prevention, quality and safety, nursing leadership, supply chain, and executive sponsors, built and implemented a process focused on establishing early, consistent interventions based on patient risk rather than relying on surveillance testing or post-colonization response.

High-risk populations were clearly defined, including critically ill patients, those with central venous access devices, patients receiving hemodialysis, burn patients, and additional cohorts as data matured. Nasal treatment strategies are now matched to patient risk, allowing escalation for select highest-risk patients while maintaining broader prevention measures for others.

Our new CHA Hospital Spotlight series continues with a feature on Ga***rd Specialty Healthcare's Sports Association 🎣🏀⛳🎾

Ga***rd Sports Association empowers people with permanent physical or visual impairments to “get back into the game of life.” What started as a basketball clinic in the Ga***rd Hospital parking lot three decades ago has grown into Connecticut’s largest adaptive sports program.

The Ga***rd Sports Association offers 17 different sports and recreation opportunities to thousands of adults with permanent physical and/or visual impairments in our state and beyond. Programs are offered for free or for a minimal registration fee. Participants do not have to have been a patient in Ga***rd’s system, although all Ga***rd Hospital patients recovering from life-altering illness or injury receive information about the program during their hospitalization.

Adaptive sports give people back more than movement … they restore confidence, purpose, quality of life, connection with others, and offer myriad long-term health benefits. It is just as intense and competitive as regular sports, and participants find the same drive and teamwork. Adaptive sport opportunities help people discover their abilities and enable participants to return to a sport they once loved, or find a new sport to connect with.

Opportunities include adaptive kayaking, waterskiing, rock climbing, cycling, archery, boccia, golf, pickleball, skiing, snowboarding, virtual boxing, yoga, Exercise for Life (high-intensity circuit training), and veterans-only programming like fishing and bowling for those with physical disabilities and/or PTSD.

Congratulations to 2026 CHA Healthcare Hero Eileen Woods, RN, MSN, Outcomes Coordinator, Quality Department 🌟 UConn Health Waterbury Hospital describes their honoree 👇

Rather than recognizing a single extraordinary moment or individual accomplishment, we have chosen one remarkable employee to symbolically represent the collective courage, resilience, loyalty, and unwavering commitment of an entire organization that refused to give up during one of the most difficult chapters in our hospital’s 130-plus-year history.

For over half a century, Eileen Woods has arrived each day with humility, compassion, professionalism, and an unshakable belief in the mission of community healthcare. Through generations of change, thousands of patients, and countless coworkers, she has remained a constant presence—steady, dependable, and deeply committed to serving others.

Eileen’s connection to UConn Health Waterbury Hospital began at just 15 years old while attending Holy Cross High School, when she joined the Candy Striper Volunteer Program. In 1979, while still a nursing student at Fairfield University, she began her career at the hospital as a trained aide. After graduating from nursing school, Eileen was hired as a registered nurse in the medical-surgical float pool and went on to build an exceptional career spanning decades. Throughout her tenure, she has served as an ICU nurse, nurse supervisor, and assistant director of the ICU and Telemetry units. She received a Master of Science in Nursing degree from University of Hartford while working as a nurse manager on the Telemetry unit. Eileen’s dedication to patient care and clinical excellence has earned her honors, including the prestigious Nightingale Award. Today, she continues to make a meaningful impact in her role as outcomes coordinator in the Quality Department, where she brings extensive nursing expertise and a steadfast commitment to quality care.
