Thanks to the Hudson Valley Times for sharing our recent announcement with Montefiore St. Luke's Cornwall, Access: Supports For Living, and Independent Living, Inc. Read more about how our Social Care Network partners are improving community health by bringing social care screenings to the ER. For a lot of people, the ER is the first time in a while they've had someone in front of them who can actually help. Now that same visit can also connect them to food, housing support and transportation.

Healthcare starts in our communities.

During National Health Center Week, we're recognizing the community health centers that provide high-quality, compassionate care to people across the Hudson Valley.

These organizations do more than deliver medical care. They connect individuals and families to the resources they need to stay healthy, from preventive services to behavioral healthcare and community support.

The Hudson Valley Care Coalition is proud to work alongside healthcare providers and community-based organizations through the Social Care Network to help eligible Medicaid members access food, housing, transportation, and other services that support whole-person health.

When patients come through the ER at Montefiore St. Luke's Cornwall (MSLC), doctors treat what's urgent. But what happens when someone doesn't have food at home? Or unstable housing?

Now, a community navigator is there too, working alongside hospital staff to connect patients with food, housing support, and transportation access before they even leave.

Part of the Social Care Network, this partnership between MSLC, Access: Supports for Living, and Independent Living is coordinated by us, Hudson Valley Care Coalition. Because health doesn't start in a doctor's office. It starts with having your basic needs met.

School's out and for a lot of families that means a summer pause for free school breakfast and lunch programs that you and your kids count on.

According to a 2026 report from the Food Research & Action Center, only about 16 out of every 100 kids who relied on free school lunch during the year are getting a summer meal replacement. That gap is real — and it's exactly what the Social Care Network exists to close.
