MISSION STATEMENT

Our mission is to passionately support the research efforts of Juvenile Diabetes Research Foundation in finding a cure for Juvenile Diabetes. We endeavor to accomplish this goal by raising awareness and funding through an annual golf tournament held in memory of Jennifer Catherine Smith (1985 – 2004). SPONSORSHIP INFORMATION

• Two (2) special tee signs

“Birdie” Hole Sponsor - $200
• Two (2) tee signs

“Par” Hole Sponsor - $100
• One (1) tee sign


Corporate Sponsorships:

* Platinum Level -- $3,000 donation

All of the items in the silver level plus your company name and logo on an upscale give-
away to golfers.

* Gold Level -- $2,000 donation

All of the items in the silver level plus an item with your company name or logo in the attendance bag given to each golfer.

* Silver Level -- $1,000 donation

10" diameter Golf Ball sponsorship sign, One team entry, display of a company banner
in the pavilion (provided by your company)
