💸 There’s a growing narrative that building GenAI-powered solutions is now “cheap.” And technically, that’s true. But here’s the catch, while models are cost effective, building healthcare solutions that have real-world impact is not.

Once you move beyond proof-of-concept and into scaled deployment, especially in healthcare, the true costs emerge. Not in base conversations, but in engagement, safety, monitoring, and continuity of care. In other words: in the things that make systems trustworthy.

🌐In 2026, equitable AI will be defined by how well it helps health systems do more with less. As donor funding landscapes continue to shift, the conversation is moving from what’s possible to what’s practical, affordable, and scalable – and success will be measured by how AI solutions respect dignity, protect privacy and solve real problems that people face.

The reflections below are shaped by our work alongside frontline health workers, partners, and communities across malaria, HIV, TB, self-care, and health system strengthening. In a year marked by even tighter resources and higher expectations, these considerations guide not just how we build AI, but how responsibly it shows up in public health systems.

And that’s a wrap on IAS 2025! 🎉

While IAS always carries a sense of collective purpose, this year’s conference hit different. There was a clear resilience among global healthcare leaders, united in the mission to make HIV prevention and care a basic human right.

This incredible community came together not just to reimagine HIV prevention but to put forward real-world innovations with the power to change the game.
AI and self-care emerged as standout themes, with the WHO’s strong recommendation of HIV self-testing (HIVST) for PrEP access cementing self-care as a critical pillar of prevention strategies.

We are proud to have presented and moderated across six AI panels, unpacking the importance of community-led AI and co-design, robust monitoring and evaluation, and building trust through safety and effectiveness.

👉🏽 Safety, equity, and cost remain top concerns for communities, implementers, and funders, who want tools that are safe and effective without requiring teams of PhD engineers.

👉🏽 From pilots to scale: Coordinated roadmaps and investments must replace “letting a thousand flowers bloom,” shifting toward scalable, monitored solutions that integrate seamlessly into national EMRs.

👉🏽 Community-first innovation: AI must be designed, monitored, and adapted with communities at the center to solve challenges rooted in their needs.

👉🏽 Tighter budgets, smarter tools: AI can build on digital successes to reduce training costs, improve health worker performance, and boost client uptake.

👉🏽 Resilience & reimagining: AI isn’t a silver bullet – but by rethinking its role, we can amplify what works and push closer to our HIV prevention goals.

📢 As the global HIV response faces unprecedented challenges due to the uncertainty around the recent Stop Order, we recognize the immense pressure our partners are under. Across the world, dedicated teams are working tirelessly to understand the impact on their programs and find ways to meet the vast needs of their communities with significantly fewer resources.

We are deeply saddened by this situation. There is no substitute for the skilled, compassionate humans who deliver critical HIV services on the ground 🎗️

In this context, we are honoured that our Self-Care From Anywhere program has been selected as one of 50 groundbreaking projects to be featured at the AI Action Summit by the Paris Peace Forum from February 10-11.

Designed to support resource-constrained clinicians, nurses, and HIV Testing Services (HTS) providers, our multi-modal AI toolkit empowers them to reach more people with high-quality, stigma-free education, information, virtual testing, and linkage to care—needs that are now more urgent than ever.

The HealthPulse AI toolkit includes:

🤳🏼AI-companion powered by an omnichannel LM service to empathetically answer questions about life, relationships, SRH, HIV, GBV, and staying healthy

🖥️ Computer vision and digital guide service for self-testing guidance and result interpretation

👩🏽‍⚕️Anonymized connections to clinicians for prevention and treatment options (human-in-the-loop support and monitoring)

📈Automated Monitoring & Evaluation service to ensure safety, flag harm, and reduce load on human labelers

🌍These services are already making an impact in programs across Kenya, South Africa, and soon Zimbabwe, helping to power differentiated service delivery programs for self-care.

NATIONWIDE DEPLOYMENT 📣 We are excited to officially announce, in partnership with Fionet Inc., the delivery of AI-powered digital health solutions nationwide in Rwanda.

Supporting Rwanda’s Ministry of Health, the delivery of HealthPulse AI embedded in Fionet Suite, a mobile health platform for primary care, will be used by over 50,000 community health workers (CHWs) and over 1,000 Health Posts in Rwanda 🙌🏽

The integration of HealthPulse AI into Fionet Suite’s point-of-care testing module will provide actionable insights into the use of rapid tests, ensure that a wide array of diagnostic results can be trusted, and inform how to best support frontline health workers.
