Meet the Parkinson’s Association of San Diego – Chula Vista Connect & Thrive one of the CVCF 2026 grant recipients.

The Parkinson’s Association of San Diego (PASD), founded in 1982, is dedicated to improving the quality of life for individuals living with Parkinson’s disease, their families, and caregivers through education, wellness programs, support groups, and community engagement. Serving more than 12,000 individuals annually across San Diego County, PASD has established strong partnerships with healthcare providers, senior centers, and community organizations to ensure accessible and equitable support.

Through the Connect & Thrive initiative, PASD will serve 150 seniors and caregivers in Chula Vista by addressing social isolation, transportation barriers, and digital inequity. Programming will take place at the Norman Park Senior Center, libraries, and other community locations to ensure broad accessibility. The program will offer twice-monthly sessions, including movement-based wellness activities such as adaptive exercise and Tai Chi, as well as digital literacy training to help participants access telehealth, online resources, and virtual support networks.

Transportation vouchers and bilingual outreach will ensure inclusion for diverse and underserved populations. In partnership with organizations such as the Chula Vista Public Libraries and San Diego Futures Foundation, the program will also connect participants to additional local resources. By strengthening social connections, improving digital skills, and promoting physical wellness, Connect & Thrive enhances independence and overall well-being for seniors and caregivers.

Meet San Diego Oasis - Oasis Access in Chula Vista one of the CVCF 2026 grant recipients.

San Diego Oasis, founded in 1987, is dedicated to promoting healthy aging by enhancing the quality of life for adults aged 50 and older through lifelong learning, health and wellness programs, and community engagement. Oasis empowers older adults to stay active, engaged, and connected.

Through Oasis Access – Chula Vista, the organization will serve 500 older adults by bringing wellness, nutrition, and social engagement programs directly into local neighborhoods. This initiative removes common barriers such as cost, transportation, and digital access by partnering with senior centers, housing providers, and transportation services. Participants will receive free wellness and cognitive health classes, digital literacy training, enrollment assistance in meal programs, and seven nutritionally balanced meals per week.

Building on a successful pilot in National City, Oasis Access aims to improve physical and mental wellbeing for at least 80% of participants while reconnecting them to essential services. By fostering social connection, improving nutrition, and supporting independence, the program helps older adults remain active in their communities, reduces strain on healthcare systems, and promotes aging with dignity and purpose.

Meet Mama’s Kitchen - Senior Nutrition Program in Chula Vista one of the CVCF 2026 grant recipients.

Mama’s Kitchen improves the health of individuals vulnerable to malnutrition due to critical illnesses such as HIV, cancer, heart failure, diabetes, and kidney disease by providing no-cost, home-delivered medically tailored meals and nutrition education. Over 35 years, the organization has delivered more than 13 million meals and is the region’s largest provider of delivered meals and the only provider of scratch-made medically tailored meals.

Through its Senior Nutrition Program in Chula Vista, Mama’s Kitchen will serve 216 low-income seniors, delivering medically tailored meals 365 days a year. Registered dietitians design each meal to meet the specific nutritional needs of clients with chronic conditions, helping improve treatment adherence and overall health. Participants also receive individualized nutrition education sessions focused on practical strategies such as reducing sodium intake and managing diets on a budget.

The AniMeals Pilot expands support by providing pet food for approximately 50 client pets, ensuring seniors do not have to choose between feeding themselves or their animals. In partnership with the Helen Woodward Senior Center, the program supports both physical and emotional well-being. This integrated, cost-effective model helps seniors maintain stability, reduce hospital visits, and age with dignity.
