What distinguishes the organization from others is that AHD is the only rural health provider in Latin America that delivers quality care based on a proven model of self-sufficiency and capacity building. AHD offers the only residency program that trains rural family physicians — where local faculty train Ecuadorian physicians to work in their own communities. Over a quarter of Ecuador's populatio

n lives on less than $4/day, and most of poor live in rural settings. Yet most of the country’s hospitals are located in the country’s two largest urban centers. Facing farm accidents, snake bites, limited childbirthing services, and living in communities without adequate sanitation, most families outside of Quito or Guayaquil have very limited access to quality health care and encounter life threatening situations all too frequently. AHD has been operating one hospital for 14 years, serving over 125,000 people. AHD has also recently constructed and opened a first class 65 bed hospital in a different part of the country. In addition to adding a critically needed hospital to an area where over 400,000 mostly poor people live, this new hospital will allow us to expand our medical training and residency program from 6 doctors a year to 70. AHD’s unique model for financial sustainability is based on strong local partnerships and keeping costs low. We were able to construct our new hospital in Santo Domingo for less than $4 million (a similar facility in the US would cost 10 times that much).

As you put together your complex holiday menu on a chilly winter day, I would like to share with you another complexity we face beyond the already challenging landscape of health care delivery: energy shortage.

Due to a severe, historic drought, the hydroelectric power that Ecuador relies on for most of its electricity has been drastically reduced. This power shortage has caused 12-hour daily blackouts.

We have always been prepared for this because electricity, even in the best of times, is suspended occasionally in our hospitals. We have powerful backup diesel generators at both hospitals to cover these moments. But the “moments” have now grown into prolonged, daily events for the foreseeable future. These generators are intentionally called “backup” because they should be used infrequently. With daily use, they require ongoing maintenance and can break down periodically. If our backup at Hesburgh Hospital breaks down during a power outage and we are left without energy, how do we keep our ventilators functioning in the ICU?

One answer is a backup for our backup generator, but Ecuador also faces the problem of a diesel fuel shortage as the entire country is purchasing diesel-powered backup generators for both domestic and industrial use. You might remember that Ecuador is a major producer and exporter of petroleum, but Ecuador does not refine the petroleum, so it must bring refined petroleum back into the country from other countries, which is difficult and expensive.

Enter stage left: solar energy.

Makes sense, right? After all, we are on the equator. We had this discussion when building Hesburgh Hospital but chose not to deploy solar at that time because the price of the equipment, when not subsidized like it is in the US, can be prohibitively expensive. Additionally, the cost of electricity was historically a fraction of what it costs in the US. Finally, the quality of the solar panels and batteries 10 years ago was not ideal.

But times have changed. Due to the scarcity (which could last at least another year or longer), the cost of electricity in Ecuador has skyrocketed. The cost of solar equipment has plunged, and the technology is also significantly better. A partially solar-powered hospital on the equator is now a viable option. “Partially” is important. Solar energy will likely not be enough to power the entire hospital 24/7. We have learned that X-ray and CT scan energy requirements are beyond what solar can offer. But solar can bridge our needs during these ongoing blackouts, and it has the advantage of reducing our ongoing energy costs, as well providing us a renewable power source for years to come. We hope that this will help keep our hospital running at all times and limit the use of the backup generator.

The cost of adding solar and a battery system is $127,000. Investments in capital equipment like this are budget items that we generally do not include in operations or consider to be part of the “self-sustainability” discussion. This is where our donors like you make the difference. Our ability to make these tough decisions and have the resources to implement them is what distinguishes us from other healthcare institutions in the Global South. Simply stated, it’s why we have been around for a long time and why, with your support, we will continue to provide high quality, uninterrupted health care to the people of Ecuador.
