Honor Roll, Honor Society, Specific School Awards, Gifted or AP Courses, etc.) and the Extracurricular Activities/Community Service: (ex. Media Day paperwork
1. Bring a good copy of your athlete’s birth certificate or the original and we will make a copy there
2. Pop Warner Contract on the registration page www.trseahawks.com print and sign
3. Pop Warner physical paperwork also on registration page this needs filled out by the parent only with insurance info. You will also need a copy of a physical from the MD on any form the MD has. The physical has to be less than 1 year old from the start of the season on August 1st. You can bring the MD portion at the start of the season
4. Report cards -not needed on media day bc kids still in school but print it out at the end of the year before the portal closes for the summer
