South Suburban Illinois Continuity of Care Association 2nd Thursday of the month of June morning meeting was held at Arden memory care in Palos. Monica Peterson detailed the group on all of the Arden locations and capabilities. Many other members also updated us on their services and capabilities for patient care. We reviewed the state meeting and all congratulated Il-COC.org on its 50th anniversary. Thanks to all of the South Suburban members and board for all you do!

Thank you Sonija Telles for hosting a delicious breakfast at the beautiful Grand at Twin Lakes!

This month the Metro West Chapter, and all of our wonderful members who were able to attend, had the privilege of a listening to an incredibly informative presentation by Legacy. Rob Glaser and John Doherty spoke to the importance of their company’s ability to curate, preserve, and share cherished family history in latest digital formats, to ensure they are not lost to time. It prompted a fun and insightful Q&A.

We invite you to join us for our next meeting on July 9 at the Sunrise of Prospect Heights. Please note that this chapter meeting will have a time change from 1-3pm so we can enjoy a fun BBQ outside!

Thank you to everyone who joined us this morning for the Kankakee-Iroquois-Livingston Continuity of Care Meeting hosted at Westwood Independent Living.

We were honored to welcome Hadi Finerty from the Alzheimer’s Association, who delivered an insightful and educational presentation on brain health. Attendees received valuable resources highlighting proactive steps we can take to support cognitive wellness and overall health. As Alzheimer's disease and other forms of dementia continue to impact so many individuals and families, ongoing education and community awareness remain more important than ever.

It was a beautiful second Tuesday of the month Illinois Continuity of Care Association, Three Rivers chapter monthly morning meeting held at the Plainfield Senior Center in Plainfield Illinois. The speaker and Sponsor was our very own Jennifer Wood-Kelly and Bobby Castillo from Care 2 You Hospice and Palliative Care. They detailed us on their services and gave us the perspective of hospice and palliative services from all viewpoints along with the history of the organization. They did a great job. Jennifer also surprised us with a home made, hot breakfast that she made from scratch herself that morning. It was one of the most tastiest meals I ever had. I really enjoy being a part of this group and all their efforts. A big thanks to all of the board members for all you do. The Illinois Continuity of Care Association had its annual 2026 meeting at the Concord in Northlake. COC is turning 50 years old and Bob McDough one of the original Presidents was our Key note speaker. Bob gave the members a history lesson on the origins of il-COC.org. This is a must see video. We also recognized Past state Presidents Mike Quirk, Pat O'Dea-evans, Jerry Schmitt and all current state Presidents. We also appreciated Celeste Basom. Happy 50th birthday COC and thanks for all you do!

What a beautiful June morning , first Thursday of the month Alpa Illinois Continuity of Care Association chapter meeting hosted by Lisle Creek Estates in Lisle Illinois. Lee Moriarty, Deputy State Ombudsman presented the duties and responsibilities of the ombudsman and the role they play as an advocate and mediator for patients in nursing homes and receiving homecare services resolve complaints. The Ombudsman is not IDPH but are here to resolve problems before they become escalated. This was a must see presentation. We also welcomed and congratulated Lorie Feeney-Catrambone as the new Alpha chapter PR board member. True impact starts with a shared mission, and that mission was on full display at the Illinois Continuity of Care Association's state meeting at Concord Place in Northlake today!

Professionals from chapters across Illinois came together for an afternoon of networking, collaboration, and leadership as incoming and returning board members were recognized, and everyone received important state level updates.

One of the highlights of the event was a presentation by legendary healthcare professional Bob McDonough, who shared a fascinating look at the history of the Illinois Continuity of Care Association. Reflecting on the organization's roots provided valuable perspective on its continued commitment to improving care transitions and strengthening post-acute care partnerships throughout Illinois.

Special thanks go to Bev Anderson and the team at Concord Place for providing a welcoming venue where attendees could connect, share a meal, and engage in meaningful conversations. Appreciation also goes to Julie Gilpin and Pete Miska, for their hard work behind the scenes in coordinating this successful event along with the rest of the state board for their dedication in advancing the mission.

The energy, engagement, and collaboration demonstrated throughout the meeting reinforced the important role the organization plays in bringing healthcare professionals together to improve outcomes across the continuum of care. Congratulations as well to the event's raffle winners! 🎁

It was an incredible morning with the Lake East chapter of the Illinois Continuity of Care Association! Thank you to our wonderful host, Andreea Ducu and her team at Warren Barr North Shore for opening their doors to us and creating such a welcoming space.

This morning wasn't your typical "swap business cards and move on" kind of meeting. Instead, we dove into an intentionally designed, highly interactive networking session. By moving past the standard pitches and asking purposeful, engaging questions, we truly got to know the people behind the titles, sparking meaningful conversations and quite a few laughs along the way.

When healthcare and senior care professionals truly connect, our entire community benefits. Thank you to everyone who showed up, leaned in, and brought such great energy to the room this morning! The Heritage Corridor leadership team made a notable arrival for the May 4th Wednesday meeting of the Heritage Corridor Illinois Continuity of Care Association, hosted by Kelly Pratscher, Executive Director, at The Reserve at Oswego, where they were joined by Kelly Pratscher's team, including the organization's President, Carl Hirschman. Kelly’s team walked us through A-Z model of how the RESERVE treats the residents, family members, and staff. Carl shared his background, beliefs systems, current and future projects and overall enthusiasm for the future. Looking forward to a wonderful summer with this group. Beautiful last Tuesday of the month, May, Fox Valley Illinois Continuity of Care Association chapter meeting. Amanda Pratt from Tower Hill Nursing Home was a wonderful host and skilled facility. The longevity of the staff and the patient care was really impressive. We had a unique presentation by our own Fleurette Elia of Journey Care Hospice on relaxation techniques that consisted of sound bowels to breathing technique. It was wonderful.
