Accountability starts with clarity. “The system failed” isn’t reporting—it’s a shortcut that erases responsibility.

Journalism must go further: trace where decisions were made, identify the structural causes (policy, design, funding, staffing), and name them plainly. Disability narratives deserve precision, not vagueness.
