Momentum at South Shore Support Services, Inc. Sharing updates about our successes and the latest developments of the community we serve! Kristine L.

As part of South Shore Support Services network of services, we are guided by the belief that the individuals we support are the directors of their support services. To that end, the agency works to empower each person to make informed life choices. We support the individual's pursuit of a quality of life, which is free from self-imposed and or external barriers. Agency With Choice is a self-direc

ted, 1:1 community service model designed to provide supports to DDS eligible adults with disabilities and their families. This gives the individual and their family authority over the design, schedule and staffing of their support service. Marquis

📢📢📢2023 MA DISABILITY COMMUNITY HEALTH NEEDS ASSESSMENT❗

We STRONGLY encourage those who see this and live with a disability to not only submit the assessment, but to share it with those you know!

In short, this assessment helps the MA Public Health Department to understand 1) the quantity of people with disabilities living within the state and 2) how someone with any given disability may be impacted and how that defines their needs.

Follow this link to the assessment: https://redcap.ehs.mass.gov/redcap/surveys/?s=J7C9M7XDH3H7ERC3

***This survey was created by the DPH and reviewed by self-advocates with disabilities.
