We're celebrating National Community Health Worker (CHW) Awareness Week! We want to recognize the invaluable contributions of CHWs in improving community health outcomes. At Health Forward, we are committed to supporting policies that respect, protect, and partner with the CHW profession. As part of this commitment, we were proud to support the Mid-America Regional Council's Practice Integration Toolkit.
