TAPCHW is proud to announce the 2026 Community Health Worker of the Year Awards, honoring the dedication, leadership, and impact of CHWs across Texas!🏆

These awards will be presented during our Virtual Conference on March 26–27, 2026, to shine a spotlight on the CHWs and CHW teams who are making a difference every single day.

Do you know a CHW who embodies the heart of this work? A rising star? A trusted leader? A team creating real community change? Nominate them!

✨ Award Categories:
🔹 Rising Star Award – CHWs with fewer than 3 years of experience who demonstrate exceptional promise, leadership, and initiative. (Individuals only)
🔹 Lifetime Contribution Award – CHWs with long-standing service and sustained commitment, leadership, and community trust. (Individuals only)
🔹 Community Impact Award – CHWs or CHW teams making a significant impact through advocacy, outreach, systems navigation, or addressing community needs. (Individuals or Teams)
🔹 Innovation in CHW Practice Award – CHWs or teams using creative approaches, tools, partnerships, or workflows to improve engagement or outcomes. (Individuals or Teams)
