A 48 year-old male patient with keratoconus came for scleral lens fitting. He came in wearing his old scleral lenses fitted from a different practice. He complained of his eyes being always red.
Upon evaluation, the patient was wearing very tight fitting lenses which caused conjunctival hypertrophy and limbal neovascularization to develop.
To enable us to have a more accurate fit, we asked the patient to stop wearing his lenses for 1 week and come back for an evaluation. Even after a week of lens wear cessation, his eyes were still very red.
He was fitted with a more customized design to remove the pressure of the haptics from his sclera. With his new lenses, best corrected visual acuity was 20/25 for OD and 20/40 for OS.
The images below are a comparison of his eyes while wearing the old lenses, and first follow-up visit after wearing the new lens for one month. We may do some more changes for the fit as his conjunctiva hypertrophy improves. Patient noted his eyes are more comfortable now, and he was happy to finally wake up with whiter eyes.
This case highlights the impact of tight-fitting lenses on the conjunctival tissue and the role of understanding the altered scleral shape in designing the lens haptics to relieve pressure on eyes with conjunctival hypertrophy.
