A 48 year-old male patient with keratoconus came for scleral lens fitting. He came in wearing his old scleral lenses fitted from a different practice. He complained of his eyes being always red.

Upon evaluation, the patient was wearing very tight fitting lenses which caused conjunctival hypertrophy and limbal neovascularization to develop.

To enable us to have a more accurate fit, we asked the patient to stop wearing his lenses for 1 week and come back for an evaluation. Even after a week of lens wear cessation, his eyes were still very red.

He was fitted with a more customized design to remove the pressure of the haptics from his sclera. With his new lenses, best corrected visual acuity was 20/25 for OD and 20/40 for OS.

The images below are a comparison of his eyes while wearing the old lenses, and first follow-up visit after wearing the new lens for one month. We may do some more changes for the fit as his conjunctiva hypertrophy improves. Patient noted his eyes are more comfortable now, and he was happy to finally wake up with whiter eyes.

This case highlights the impact of tight-fitting lenses on the conjunctival tissue and the role of understanding the altered scleral shape in designing the lens haptics to relieve pressure on eyes with conjunctival hypertrophy.

An 11-year-old male with a history of corneal perforation repair, lensectomy, and vitrectomy
presented with aphakia and visual acuity of CF @ 1 feet. The fellow eye had 20/20 vision. A soft
contact lens trial did not improve vision due to the irregular corneal surface. A scleral lens was fitted with minimal vault to maximize oxygen permeability considering the compromised endothelium, improving visual acuity to 20/100 and providing functional visual rehabilitation. This case highlights the role of scleral lenses in managing complex post-surgical aphakia with irregular cornea.

A recently diagnosed 25 year old male attended for a scleral lens evaluation. Unfortunately, due to the advanced nature of the keratoconus and advanced thinning of the cornea, corneal collagen cross linking was not an option. This patient was listed for a penetrating keratoplasty but wanted to explore non-surgical options available to him. Presenting vision in the right eye was counting fingers, with a bet corrected acuity of 20/400. A free form scleral lens was designed for this patient and he achieved corrected acuity of 20/30, avoiding the need for corneal surgery.

Poor peripheral alignment can lead to redness, vascular compression, discomfort, hypoxia, or even lens discontinuation.

Every detail matters. A well-fitted scleral lens shouldn’t just look good centrally, it must also respect the ocular physiology in the periphery.
