Games and Activities will be provided by Little Sprouts Enrichment Center

For OSF KING CARE-A-VAN
Please bring:
*Permission slips for family members whose student will not have a parent present
*IHSA forms (if you have them)
*Insurance card if billing insurance

Physicals will be $35.00, no cash will be accepted. Credit cards may be used for any copay. Medicaid will be billed with no copay day of service

Note: No vaccines will be given at this event
