Medical, Urgent Care, Dental, Mental Health, Substance Use Disorder Treatment, Podiatry, Optometry & Optical Shops, Pediatrics and Health Insurance Enrollment Assistance Community Health Connections (CHC) is a multi-site, non-profit health care center offering urgent care, primary family medical and pediatric care, preventative and restorative dental care, oral surgery, mental & behavioral health services for children and adults, Psychiatry, Health Insurance Enrollment Assistance, substance use disorder treatment, and specialty services including optometry eye care, optical shops, nutrition consultations, podiatry, TMS Clinic, Spravato Clinic and Outpatient Day Treatment. CHC has five sites within Fitchburg, Gardner and Leominster with decades of experience as a Federally Qualified Health Center (FQHC), serving 36 communities in North Central Massachusetts. All CHC health centers are located on the Montachusett Regional Transit Authority bus line. Community Health Connections was established in 2002 and is a Federally Qualified Health Center (FQHC). Community Health Connections (CHC) is a mainstay for compassionate care, regardless of income or insurance status. Our mission is to provide high quality, affordable health care in our community. Our vision is to be the leading health care center, committed to eliminating barriers to care and building a healthier tomorrow for the communities we serve; Acton, Ashburnham, Ashby, Athol, Ayer, Baldwinville, Barre, Boxborough, Clinton, Devens, East Templeton, Fitchburg, Gardner, Gilbertville/Hardwick, Groton, Harvard, Hubbardston, Lancaster, Leominster, Littleton, Lunenburg, New Braintree, Pepperell, Phillipston, Princeton, Orange, Royalston, Rutland, Shirley, Sterling, South Lancaster, Templeton, Townsend, Westminster, West Townsend, and Winchendon. Our community of patients includes insured, underinsured, uninsured, low income, publicly housed, and homeless populations.

Oh my gourdness, it’s time for Growing Places Mobile Farmers Market stop at CHC.  Grab a friend, visit the farmers market in our main lobby, have lunch at the Finicky Fork Cafe located on our second floor, and then head outside for walk around campus or at Mass Audubon’s Flat Rock Sanctuary or North County Land Trust’s Crocker Conservation Area. It’s supposed to be beautiful day ☀️ 🍁

Before you go, know where to go. www.chcfhc.org/urgent-care Choosing the right place for care can help you get the appropriate treatment you need, while ensuring hospital emergency resources are available for those who need them most. CHC Urgent Care & Virtual Urgent Care is a great option for many non-life-threatening illnesses and injuries, including cold and flu symptoms, fever, sore throat and earaches, and minor cuts, sprains and other minor injuries. CHC operates 3 Urgent Care Centers and now offers Virtual Urgent Care via MyChart. Walk-in or Connect online. Open to the public. Expanding, Growing, Hiring, Serving! Thank you, Congresswoman Lori Trahan, Senator John J. Cronin, State Representative Mike Kushmerek, Sam Squailia, Fitchburg Mayor for stopping in to see the new space we are building out in Fitchburg.🚧🧰🔨

Since it opened, Community Health Connections’ Children's Behavioral Health Center in Fitchburg has served 1,403 young patients during 25,815 visits.

Now they're nearly doubling the space to add more rooms, a new youth day treatment program, and shorten waits for families.

I’m proud of the work we did to secure $2.5 million in federal funding to help staff this center, and I’m grateful for the opportunity to see what that investment looks like up close today!
