preventative and restorative dental care, oral surgery, mental & behavioral health services for children and adults, Psychiatry, Health Insurance Enrollment Assistance, substance use disorder treatment, and specialty services including optometry eye care, optical shops, nutrition consultations, podiatry, TMS Clinic, Spravato Clinic and Outpatient Day Treatment. CHC has five sites within Fitchburg, Gardner and Leominster with decades of experience as a Federally Qualified Health Center (FQHC), serving 36 communities in North Central Massachusetts. All CHC health centers are located on the Montachusett Regional Transit Authority bus line. Community Health Connections was established in 2002 and is a Federally Qualified Health Center (FQHC). Community Health Connections (CHC) is a mainstay for compassionate care, regardless of income or insurance status. Our mission is to provide high quality, affordable health care in our community. Our vision is to be the leading health care center, committed to eliminating barriers to care and building a healthier tomorrow for the communities we serve; Acton, Ashburnham, Ashby, Athol, Ayer, Baldwinville, Barre, Boxborough, Clinton, Devens, East Templeton, Fitchburg, Gardner, Gilbertville/Hardwick, Groton, Harvard, Hubbardston, Lancaster, Leominster, Littleton, Lunenburg, New Braintree, Pepperell, Phillipston, Princeton, Orange, Royalston, Rutland, Shirley, Sterling, South Lancaster, Templeton, Townsend, Westminster, West Townsend, and Winchendon. Our community of patients includes insured, underinsured, uninsured, low income, publicly housed, and homeless populations.

Parents - Keep an eye out! CHC's Caring for Kids Mobile School-based Dental Program registration packets are coming home from your child's school. (See listing below for contracted school districts.)

Our school based dental program makes it easy for your child to get the dental care they need, right at school!

Services include; Dental Exams, Teeth Cleanings & Fluoride Treatments, X-rays & Sealants, SMART restoration, SDF (Silver Diamine Fluoride) and dental education. There is no cost to families. Every school age child is eligible to participate, regardless of dental insurance status or ability to pay. (If your child does have insurance, it will be billed).
