Since launching in 2018, the Bridges Program has supported thousands of young adults as they transition from foster care into adulthood. Today, we’re proud to share a major milestone: more than 1,000 young adults actively participating at the same time, which is a 35% increase over past years.

This growth reflects intentional outreach, stronger transitions for young people aging out of care, and deep collaboration with provider partners and public children services agencies across Ohio. When access is clear and support is strong, young people show up and thrive.

We’re grateful to our partners and providers, and especially to the young adults who trust Bridges as they build stability, independence, and their next chapter.

Recently, the Child & Family Health Collaborative hosted a System of Care Workshop focused on strengthening support for youth and families during the transition from inpatient behavioral healthcare.

Facilitated by Jeff Steller from Aetna OhioRISE, the session brought together partners from community mental health agencies, county and state agencies, and children’s hospitals. Using LEGO Serious Play, participants explored innovative ways to improve post-discharge success and help stabilize families during high-risk transitions.

We’re grateful to the partners who came together to strengthen Ohio’s system of care for youth and families. 🤝
