❤️ Making a Difference One Family at a Time ❤️
At Bridges to Wellness, we are honored to walk alongside families as they navigate challenges and connect with the resources they need to thrive. This program is made possible, in part, by The United Way of Muskingum, Perry & Morgan Counties
Recently, we received a heartfelt letter from a mother who shared:
"The reason for this letter is to thank you immensely for the help and support that you have given us."
Through the support of Stefanie a CHW out of Zanesville-Muskingum County Health Department, this family was able to access children's health insurance, food assistance, transportation to WIC appointments, and a payment plan for medical services.
"My family is grateful for your and Stefanie's help and support."
Stories like this remind us why care coordination matters. When families have someone to help them navigate healthcare, social services, transportation, and other essential resources, barriers become opportunities and challenges become successes.

💜 CHW Day Spotlight: What are you Proud Of? 💜

When asked, "What are you proud of?" Stefanie's answer was simple but powerful:

"Gave hope when there was none."

As a Community Health Worker out of Zanesville-Muskingum County Health Department, Stefanie walks alongside families during some of their most challenging moments. Through support, encouragement, and connecting families to vital resources, she helps ensure that no one has to navigate difficult circumstances alone.

Hope can be life-changing. Sometimes it looks like finding transportation to an appointment. Sometimes it's connecting a family to community resources. Sometimes it's simply knowing someone cares.

Tabatha Jackson, BTW regional coordinator, participated in a panel discussion regarding “difficult conversations with clients”, at the Legal Aid of Southeast and Central Ohio - LASCO conference today. 130 attorneys and staff members were in attendance and asked a lot of questions on how to address emotional clients and still meet their needs while providing legal services. Great discussions were had. Thank you so much for including our Regional Coordinator!!
