PCC Community Wellness Center PCC is a network of 14 health centers on Chicago's West and Northwest Sides and in the near west suburbs.

We offer primary care, behavioral health care, and dental care. The mission of PCC Community Wellness Center is to improve health outcomes for the medically underserved community through the provision of high quality, affordable, and accessible primary health care and support services. Anchored with family medicine, we are committed to serving the needs of all people in all stages of life. PCC ori

ginated in 1980 as the Parent Child Center, a three-room clinic of West Suburban Medical Center that offered prenatal, postpartum, and infant care for residents of Chicago’s Austin community. In 1992, PCC was incorporated as an independent, 501(c)(3) nonprofit organization. PCC attained the status of a Federally Qualified Health Center (FQHC) look-alike in 1994 and received full FQHC recognition in 2002. PCC now comprises 14 sites that deliver comprehensive health care and support services on Chicago’s West and Northwest Sides and in the near west suburbs. PCC envisions a future in which our society has developed a Culture of Health that guides our path to achieving and maintaining the healthiest population possible, and that guides our actions for providing all individuals and communities with the means to attain the best health possible.

During Mental Health Awareness Month, the Village is highlighting community partners that strengthen access to care and support for residents across Oak Park. Organizations such as Oak Park River Forest Infant Welfare Society, PCC Community Wellness Center, Mosaic Counseling and Wellness, LLC, Thrive Counseling Center (THRIVE) and NAMI Metro Suburban serve as vital connectors to mental health, wellness, crisis response and family support resources for individuals of all ages. The Village’s E.C.H.O. team also helps residents navigate available support and connect with services that meet their needs. Whether someone is seeking counseling, substance use resources, or assistance identifying the next step, E.C.H.O. works to ensure that support remains accessible, coordinated and community centered. For more information, visit www.oak-park.us/ECHO or contact the E.C.H.O. team at 708.358.5640 or [email protected].
