“Integrity.” “Joy.” “Everything.”

We asked our incredible community health workers what serving their community means to them, and their answers say it all. 💙

Every day, they show up with compassion, commitment, and care to help build a healthier South Side. We could not be more grateful for the team that makes our work possible.

Most of what shapes your health happens outside the doctor's office, like access to stable housing, healthy food, reliable transportation, and more.

Reginald was navigating all three at once when he came to SSHCO. Swipe to learn his story.

Coordinated care and social support give patients somewhere to turn before things get bad. That has a ripple effect on the entire healthcare system.

Did you know that health outcomes are shaped by more than just access to medical care?

The social determinants of health — like housing, food access, transportation, and economic stability — play a major role in overall well-being and can directly impact a person’s ability to receive and benefit from care.

Our CEO, Kimberly Hobson, shares how SSHCO approaches healthcare through this broader lens. By addressing both clinical needs and the social factors that influence health across the communities we serve, SSHCO helps bridge gaps and ensure people have the resources they need to thrive.

Happy Mother’s Day to all the mothers on the South Side!

The journey to motherhood brings challenges that no one should have to navigate alone. Today, we’re sharing the story of Keyonna Tompkins, our Program Manager for Maternal Health & Chronic Disease, whose personal experience reminds us why wraparound support matters for mothers.

During her own high-risk pregnancy, Keyonna experienced symptoms she thought were “normal” and didn’t speak up about it. She soon went into labor without realizing it and gave birth 13 weeks premature. Today, she uses her story to empower other mothers to trust their instincts and ask questions.

At SSHCO, we’re here to support you before, during, and after your pregnancy.

Spring has sprung! 🌷☘️

This month, our team of Community Health Workers has been at events across the South Side, bringing our resources directly to the community.

An event that was particularly special was “Spend in the Black,” spearheaded by Alderman Will Hall of the 6th Ward, which brought together over 100 Black businesses and organizations for a day of excitement and pride.

We were thrilled to participate as vendors alongside Walgreens at this celebration of Black business and community.

Thank you to everyone who stopped by to meet our staff and get connected to resources for your health and wellness!

Meet Tasha, one of SSHCO’s Nurse Care Coordinators, who shares how SSHCO was able to assist a gunshot wound patient and his grandmother through his journey of healing and recovery.

Tasha helped him receive the physical and behavioral health care he needed, made sure he received the right medication, and connected him to various social supports. With SSHCO’s help arranging transportation, he didn’t miss a single appointment and is now able to live a happier and healthier life. 💙

Get outside, Chicago! 🚶🏃🏼🧑‍🦽

Regular time outdoors helps lower blood pressure, regulate blood sugar, boost your mood, and lower your stress. Even a short daily walk can make a real difference.

This is Nicole’s story. She was struggling to get to appointments and meet regularly with a doctor. Then she met Lunye, one of SSHCO’s Community Health Workers, who not only connected her to the right resources, but showed up for her when she needed it most.

Nicole’s story is just one example of the impact of our outreach and care coordination services. We’re proud of our staff who show up every day for the South Side. 💙
