ComServ, Inc. is a Certified Care Management Agency by the North Carolina Department of Health and Human Services. Tailored Care Management is offered at no cost to individuals who will be served by North Carolina’s Behavioral Health and Intellectual/Developmental Disability (I/DD) Tailored Plans and for some individuals who will continue in Medicaid Direct.
Tailored Care Management (TCM) provides members with a designated care manager that will assist the member and/or their families to coordinate services for their son/daughter/family member. Care Managers will develop a multidisciplinary care team to provide whole-person care for the individual served. Whole person care includes helping to meet the following needs of the individual: physical health, pharmacy, long-term services and supports, unmet health-related resource needs, housing, nutrition, education, medically necessary transportation and many other areas . You now have someone to call to assist you with getting connected to available resources!
