The GUIDE Program was created to support both individuals living with dementia and the caregivers who care for them every day. From care coordination and education to 24/7 hotline access and respite care, this Medicare-supported program is designed to help lighten the load and improve quality of life.

If you are caring for someone with dementia, the GUIDE Program may be able to provide support at no additional cost.

Eligible participants can receive caregiver education, ongoing support throughout the dementia journey, possible respite care, and 24/7 access to a care team member — all without changing their current doctors or care providers.

Even individuals without a caregiver may still qualify if they meet eligibility requirements.
