Chestnut Family Health Center is a federally-qualified, community-based health center that offers care for all ages with locations in Bloomington, Belleville, Granite City, and Maryville, Illinois. Our team is here to provide you and your family with high-quality care that meets your needs. As part of Chestnut Health Systems, you’ll also have easy access to mental health and psychiatry services, p

rimary care, lab tests, a pharmacy, community resources, and more. We’re here to support your health so you can live fully! Chestnut is a Patient-Centered Medical Home, which means a team of doctors, nurses, and other health experts will work together to take care of you. You are the most important part of the team! It’s important to tell us about your health and any concerns you have. This includes changes in how you feel, the medicines you take, any trips to the emergency room or hospital, visits with other doctors outside of Chestnut, and medical tests done elsewhere. Chestnut is a 501(c)(3) not-for-profit organization.
