Recommendations:
1. *Limit salt intake*: The American Heart Association recommends consuming no more than 2,300 milligrams of sodium per day

Executive Summary:
This proposal outlines the establishment of a face book page and WhatsApp channel aimed at supporting elderly individuals in rural areas struggling with hypertension and diabetes. Our objective is to provide essential medical services, education, and support to improve their health outcomes.

Background:
Rural elderly populations often face significant barriers in accessing healthcare services, particularly for chronic conditions like hypertension and diabetes. This initiative seeks to bridge that gap by providing regular medical support, education, and monitoring.

Objectives:
1. Provide monthly medical support, including antihypertensive and antidiabetic medications.
2. Supply blood glucose machines (glucometers) and blood pressure machines.
3. Train caregivers on using medical equipment.
4. Conduct weekly health monitoring and compare results with caregiver reports.
5. Educate elderly individuals on medication adherence and disease management.

Methodology:
1. Establish a WhatsApp channel for communication and coordination.
2. Identify and recruit local healthcare professionals for monthly visits.
3. Source and distribute medical equipment and medications.
4. Develop and implement a training program for caregivers.
5. Conduct regular monitoring and evaluation.

- Training sessions twice a week for 2 years, at 300k per session: 374,400,000 UGX (calculated as 2 sessions/week * 52 weeks/year * 2 years * 300k)

3. Medications and Supplies
- Hypertensive drugs at 20,000 monthly for 2 years for 10,000 patients: 4,800,000,000 UGX (calculated as 10,000 patients * 20,000 * 24 months)
- Diabetic drugs at 20,000 monthly for 2 years for 10,000 patients: 4,800,000,000 UGX (calculated as 10,000 patients * 20,000 * 24 months)

4. Operational Costs
- Transport costs at 150,000ugshs monthly for 2 years: 3,600,000 UGX (calculated as 150,000* 24 months)

We visit the elderly people monthly providing them with antihypertensive, antidiabetic drugs, blood glucose machines(glucometers) and blood pressure machines.
Training the care takers on how to use the blood glucose machines and the blood pressure machines.
