服務對象:設籍及實際居住新北市新莊區、泰山區、林口區、五股區、淡水區、三芝區、石門區、金山區、萬里區等九區之身心障礙朋友及其家屬
服務內容：
1.針對弱勢身心障礙朋友個別需求，提供多元完善的社會福利服務管理計畫
2.電話關心身心障礙朋友福利需求與生活
3.專業服務人員到宅關懷問候及陪伴
4.辦理各類創新多元專案活動，豐富身心障礙朋友心靈與生活
5.辦理各種身心障礙照顧者支持性團體活動，具體支持辛苦的照顧者家屬朋友
6.獨居弱勢、行動不便的身心障礙朋友關懷訪視服務
7.民國104年6月3日集眾人之力，成立合唱團，發揮弱勢身心障礙者天賦歌聲潛能及生命的堅強韌性，見證鼓勵社會大眾
