弘道潮州站 - 服務內容
●服務對象： 潮州鎮內年滿65歲以上獨居弱勢長輩為主，需經社工評估，長者是否需要提供服務。
●服務內容： 1.關懷訪視 2.電話問安 3.量血壓服務 4.剪髮服務 5.維修服務 6.弱勢長者住宅修繕（以長者住宅安全為考量，需經評估） 7.大掃除（經評估可予以協助者） 8.代辦事項（協助行動上較不方便外出的獨居長輩，使其能維持正常的日常生活） 9.節慶送禮（母親節、端午節、父親節、中秋節等節慶） 10.送年菜（弱勢獨居長者） 11.送物資（弱勢獨居長者） 12.辦年貨（弱勢獨居長者） 13.辦理重陽敬老活動 14.轉介服務 15.不老樂團培育方案
