I am grateful to all the local multi-disciplinary team members for their enthusiasm, cooperation, and participation in the above clinics. With their efforts we are now able to offer free genetic testing for CFTR mutations at PIMS last, Islamabad. We are hoping to start sweat testing at PIMS soon also. Free genetic testing is already available at Indus Hospital Karachi, where sweat testing will be available soon also, Inn Shaa Allah.

Trikafta is currently being given free to the eligible patients in Indus Karachi, Indus Lahore and Lahore Children’s Hospital. This is likely to be extended to PIMS Islamabad, Inn Shaa Allah

We will continue to expand the CF services in other parts of Pakistan. Faisalabad and Sargodha in Punjab are already under consideration. Any medical colleagues willing to cooperate and set up CF services in their locality, please liaise with myself.

New research from the University of California, Los Angeles (UCLA) has revealed a promising step toward a potential one-time gene therapy for cystic fibrosis.

Using a non-viral gene-editing approach, researchers successfully delivered a healthy copy of the CFTR gene into human airway cells using lipid nanoparticles - restoring up to 88–100% of normal CFTR function in early laboratory tests.

Importantly, this strategy is designed to work across many different CF mutations, offering hope for the approximately 10% of people with CF who do not benefit from current CFTR modulators.

While this research is still in the early stages, it represents an exciting proof of concept and a meaningful step toward more universal, long-lasting treatment options.

For families currently without effective therapies, this kind of progress matters. It shows a path forward.

I big thank you to the whole CF Team at PIMS for conducting the CF Clinic at PIMS last Tuesday. This was led by Dr Nadia Waheed (Consultant Paediatric Gastroenterologist at PIMS), Dr Kanwal Jahenzeb (Paediatric Pulmonologist at CMH Rawalpindi), Dr Zainab (Nutritionist at Shifa Hospital Islamabad), Dr Bisma (Physiotherapist at PIMS).and supported by Dr Maryam and Dr Tariq from PIMS.

Clinic was well attended by the CF families from many different parts of Northern Pakistan and all had given a positive and encouraging feedback.

I am grateful to the admin of PIMS for facilitating and providing the necessary venue.

We look forward to making this a regular occurrence and once such a facillty is established, Trikafta will ne available from PIMS, Inn Shaa Allah.

Happy New Year to All CF families

2025 brought hope to many CF families in Pakistan, as Trikafta was made available to eligible patients over 6 years - thanks to the Direct Relief charitand. This would not have been possible without the help and devotion of olleagues at Indus Hospitals (Karachi and Lahore) and Lahore Children’s Hospital. Many thanks to the volunteers who helped with organising the CF clinics across the country, supported genetic testing and collaborated with international CF organisations. We are determined to continue to work to improve CF services in Pakistan, secure Trikafta for children between ages 2-6 years, and make supply of CF medication easy and affordable.

Heartfelt condolences to families who lost their beloved ones in 2025 - Inna lillahi wa inna alaihi raje'oon. May Allah Ta'aala bless them with sabr e jameel and ne'mal badal. Aameen summa aameen

MWAP held a focused webinar titled *Living with Cystic Fibrosis in Pakistan: Challenges, Hope, and Resilience* on 21 November 2025. The session was organized by Dr. Wajiha Rizwan and moderated by Prof. Dr. Saima Batool. Prof. Iqbal Bano delivered an insightful overview of the genetic basis, diagnostic challenges and clinical features of Cystic Fibrosis in Pakistan. Participants were briefed on the importance of early suspicion, the role of sweat chloride and genetic testing, and the need for timely referral to specialized centers, especially as targeted therapies such as DNase and CFTR modulators including Trikafta are now available in Lahore and Karachi due to effort of Children's hospital Lahore pulmonology department, Dr. All Akbar, Malik sb n VC UCHS, Prof. Masood Sadiq sb.

The discussion also covered evidence based management, nutritional support, airway clearance techniques and appropriate use of nebulized antibiotics. Questions from residents and faculty focused on early signs, differentiation from asthma and recurrent pneumonia, availability and cost of new therapies and referral pathways. The webinar strengthened understanding of Cystic Fibrosis among trainees and clinicians, and highlighted growing national and international collaboration to expand services across Pakistan. MWAP acknowledges the valuable contribution of Prof. Iqbal Bano and the active participation of attendees.
