📌 Please read carefully before filling out the form:
• Only the first 5 students who successfully submit this form will be entertained (1 student per family).
• Submission of this form does not automatically guarantee a slot.
• A slot will only be secured if the student submits the complete requirements IN PERSON by Monday, 9:00 AM.
• If the student fails to submit the requirements on time, the slot will be given to another student.

1. Barangay Indigency (Purpose: Educational Assistance – School Supplies)
2. Photocopy of Student ID

🎒📚 Educational Assistance - School Supplies Alert! 📚🎒

Since malapit na ang pasukan, The Perpetual Youth Organization, through the help of Pag-asa Youth Association of the Philippines – Las Piñas, is excited to share a little help with our fellow students! ❤️

We were given limited slots only, but we still want to share this small blessing by giving out school supplies to the first 5 students (ages 15–19, Grades 7–12) who can complete the requirements. 🎁

📝 Google Form will be posted TOMORROW (June 14, 2025) at 9:00 AM here on our page – first come, first served!
📌 Only 1 student per family
⏳ Deadline to submit complete requirements: Monday, 9:00 AM
🚨 Incomplete or late submissions will result in forfeited slots.
