Bridging medicine, empathy, and family-centered care. ๐ฉบ๐
Our Deanโs Meeting yesterday (July 29, 8:30 AMโ2:00 PM) was an intensive, high-yield defense centered entirely on the essence of holistic healthcare: the Patient-Family-Community (PFC) Matrix.
We opened the session with a prayer and immediately advanced to our primary focusโthe Family Case Presentation of Patient A.M., a 47-year-old furniture maker from Consolacion.
What began as a consultation for persistent glove-and-stocking paresthesia and non-healing lacerations unfolded into a multi-layered clinical challenge. Through comprehensive examination, our clerks uncovered a deeply complex diagnostic picture:
Physical & Endocrine: Poorly controlled Type 2 Diabetes Mellitus complicated by Diabetic Peripheral Neuropathy and Loss of Protective Sensation (LOPS).
Psychosocial & Behavioral: Severe Alcohol Use Disorder, Moderate Ni****ne Dependence, Major Depressive Disorder, and Generalized Anxiety Disorder.
Gastrointestinal: Chronic Reflux-Type Dyspepsia.
As Diamond Physicians, we demonstrated that healing requires looking far beyond basic laboratory values. A critical cornerstone of our management plan was addressing the patient's Emotional Critical Misperceptions (ECM). His fears regarding medication toxicityโspecifically his anxiety over metformin causing renal harmโdirectly triggered his non-compliance and subsequent chronic decline. By prioritizing targeted counseling and motivational interviewing, we dismantled these deep-seated medical misperceptions. This strategic approach allowed us to rebuild trust, advocate for strict medical compliance, and establish a safe, holistic psychiatric and physical recovery pathway within his family dynamic.
Following this, we transitioned into our Journal Presentation and concluded with a Small Group Discussion (SGD) reflection on our clinical immersion at the Kauswagan Community Clinic and Social Center.
Our panel of mentors provided invaluable critiques that will sharpen our practice as we continue to care for marginalized families.
We extend our deep respect and gratitude to our esteemed evaluators for steering our medical growth:
Dr. Enrico Gruet (Our Dean)
Dr. Chloe Parilla (Chairman of Kauswagan Community Clinic and Social Center)
Dr. Rodrigo Monleon (Resident Doctor)
Dr. Eunice Tiu (Clinical Preceptor)

Case defended, roles integrated, and our commitment to community healthcare renewed. ๐ค
