Kindly submit the following requirements to be eligible for the scholarship grant:
1. CERTIFIED TRUE COPY OF GRADES
• 1st Semester, 2025-2026
2. CERTIFIED TRUE COPY OF CERTIFICATE OF REGISTRATION
• 2nd Semester, 2025-2026
• with breakdown of fees
3. ORIGINAL COPY OF LATEST CERTIFICATE OF EMPLOYMENT
• Additional requirement for Master’s and Doctorate Studies only

For those who will submit the requirements THROUGH AN AUTHORIZED REPRESENTATIVE, also submit the following:
1. Authorization Letter
2. Photocopy of a valid ID of the:
• Scholar
• Authorized Representative

To complete the screening process and be officially considered as a QUALIFIED SCHOLAR, the qualified applicant must submit a CERTIFIED TRUE COPY OF HIS/HER GRADES for the subjects per Certificate of Registration (COR) submitted for the scholarship application (no failing/incomplete grades or dropped subjects).
