*Objective:* Provide psychosocial support and Physiotherapy home-based care to individuals in need within the community.

*Team Plan:*

1. *Psychosocial Support:*
- Conduct community outreach and identify individuals in need of support.
- Provide counseling services and connect individuals with relevant resources.
- Organize group therapy sessions and support groups.
2. *Physiotherapy Home-Based Care:*
- Identify individuals requiring physiotherapy services.
- Develop personalized treatment plans and provide regular home visits.
- Monitor progress and adjust treatment plans as needed.
3. *Collaboration and Referral:*
- Work with local healthcare providers and organizations to ensure comprehensive care.
- Refer individuals to specialized services when necessary.
4. *Education and Awareness:*
- Conduct community education sessions on psychosocial support and physiotherapy services.
- Promote awareness on the importance of mental and physical health.
