Close to 300 participants joined KESHO and the Kenya Cardiac Society for a case-based discussion on protecting cardiovascular health across the cancer treatment journey.
The discussions brought several practice-changing issues into sharp focus:
𝟏. 𝐂𝐚𝐫𝐝𝐢𝐨𝐭𝐨𝐱𝐢𝐜𝐢𝐭𝐲 𝐢𝐬 𝐛𝐫𝐨𝐚𝐝𝐞𝐫 𝐭𝐡𝐚𝐧 𝐡𝐞𝐚𝐫𝐭 𝐟𝐚𝐢𝐥𝐮𝐫𝐞.
Cancer therapy may be associated with arrhythmias, hypertension, thrombosis, myocardial ischaemia, myocarditis, pericardial disease, valvular disease and cardiomyopathy.
𝟐. 𝐁𝐞𝐠𝐢𝐧 𝐛𝐞𝐟𝐨𝐫𝐞 𝐭𝐡𝐞 𝐟𝐢𝐫𝐬𝐭 𝐝𝐨𝐬𝐞.
Cardiovascular risk factors, existing heart disease, previous cancer treatment and the proposed treatment regimen should guide baseline assessment.
𝟑. 𝐃𝐨 𝐧𝐨𝐭 𝐝𝐢𝐬𝐦𝐢𝐬𝐬 𝐚 𝐫𝐞𝐬𝐨𝐥𝐯𝐞𝐝 𝐬𝐲𝐦𝐩𝐭𝐨𝐦.
Chest pain, breathlessness, palpitations, unexplained bradycardia or a change in functional status may require evaluation before treatment continues.
𝟒. 𝐀 𝐧𝐨𝐫𝐦𝐚𝐥 𝐞𝐣𝐞𝐜𝐭𝐢𝐨𝐧 𝐟𝐫𝐚𝐜𝐭𝐢𝐨𝐧 𝐝𝐨𝐞𝐬 𝐧𝐨𝐭 𝐚𝐥𝐰𝐚𝐲𝐬 𝐭𝐞𝐥𝐥 𝐭𝐡𝐞 𝐰𝐡𝐨𝐥𝐞 𝐬𝐭𝐨𝐫𝐲.
Global longitudinal strain, ECG findings, cardiac biomarkers and the clinical context can reveal abnormalities before overt heart failure develops.
𝟓. 𝐒𝐮𝐫𝐯𝐞𝐢𝐥𝐥𝐚𝐧𝐜𝐞 𝐝𝐨𝐞𝐬 𝐧𝐨𝐭 𝐧𝐞𝐜𝐞𝐬𝐬𝐚𝐫𝐢𝐥𝐲 𝐞𝐧𝐝 𝐰𝐢𝐭𝐡 𝐜𝐚𝐧𝐜𝐞𝐫 𝐭𝐫𝐞𝐚𝐭𝐦𝐞𝐧𝐭.
Some cardiovascular complications may emerge years later, particularly following anthracyclines, chest radiotherapy, stem cell transplantation and combination therapies.
