UPDATE: Motorcycle driver reports that Shouby was admitted to the hospital last night.
11 year old Shouby's dad arrived with him in his arms. He had a seizure this afternoon for the first time. There was no history of head trauma and no fever. We got a urine specimen that showed lots of blood and protein which is an indicator of strep damage to the kidneys. While working with him he had another seizure. His blood pressure was 190/120 at its highest. Fortunately, his kidney test showed no lasting damage as yet. Please pray for Shouby as Dad is taking him to the emergency room at St. Boniface tonight on a motorcycle. They had nothing for hospitalization. We gave him several injections of anti seizure meds as well as 1 anti hypertensive medication. Shouby never regained normal consciousness for us. He was combative at times when "awake". Please pray for this young man and his dad as well as the moto driver and staff at the hospital to know how to treat him. Thanks!

Gangs have done a tremendous amount of damage to Haiti. In the next few weeks I will try to bring you up to date with some of those changes which have impacted how we care for our patients.

2 of the biggest hospitals for the poor were attacked and are now devoid of equipment and staff.

HUEH was the state hospital and was the main training center in Port au Prince for medical workers: doctors, dentists, nurses, pharmacists, etc. It had been renovated and added on. It was basically free care for the poor and many of the poor that had difficult things to treat found care there. After the 2010 earthquake they were packed. We sent folks there when no one else would accept them.

HUM in Mirabalais was a huge campus that was also a training center. Dr. Paul Farmer a cofounder of Partners in Health helped to make it possible with advanced technology including extensive solar power. They had cancer treatment and many specialties. Back when we had an air ambulance we often sent patients there when no one else could help.

These were a huge loss not only for their medical care of the poor, but for the loss of medical training facilities.

A young diabetic on insulin came in seizing while Dr. Jim was in the OR operating. Our ER nurses did a glucose check which was very low and put an IV in with high glucose injections in minutes. She woke up in just a few minutes. Dr. Jim instructed as he was informed what they should do: all is well without him needing to leave the OR. I am so impressed with our nurses Lorette and Marlene!!! Apparently she took insulin and did not eat immediately.

This is our second patient with a transfusion this week. This woman has had severe anemia from vaginal bleeding while taking good doses of iron. Hormones to stop bleeding have not worked well and the iron is not increasing her blood count. We did a bone marrow biopsy today and were unable to get any specimen to send in. Perhaps this is her problem.

Some Updates for our prayer partners:
1. Germil who fell out of a cherry tree and is paralyzed from the neck down is doing OK. He did not like the cervical collars we tried to get him to wear to prevent further extending the paralysis by turning or bending his head too much. Praise God his condition is slightly better with some increased strength in his hands. We have asked for a donated wheelchair for him.
2. Eunide with the critical infection under her chin was admitted to a hospital that night. We started antibiotics prior to departure on a motorcycle.
3. Jonathan has had 2 chemotherapies and most of his lymph nodes are no longer noticeable. Burkitt's lymphoma has a high cure rate with chemo.
4. We are completely out of rabies vaccine and tetanus immune globulin. Both rabies and tetanus are 100% fatal if not treated. Please pray that we can get the vaccine from the Haitian health department (I heard there is none in the country). We got the tetanus immune globulin in a donation.

UPDATE on Guyson: He returned to us in good health. They did not do surgery and unfortunately this condition can return if they do not remove the problem with surgery. Yesterday he had an asthma attack, but did fine.

Please pray for Eunide. She is 26 weeks pregnant and has a serious infection under her tongue that could block her breathing. We just sent her to the hospital. Pray that she can continue to breathe and that the antibiotics will decrease the swelling quickly. Thank you!

Please pray for Elina who is 7 months pregnant. She began bleeding heavily this morning and her family came for a letter to go to the delivery center this morning at 7. If true, this is a medical emergency.

A 10 year old boy named Guyson, has been having abdominal pain off and on since October in the vicinity of the appendix, but it is not the appendix. We sent him once and they didn't do surgery. Recently he has come back several times a week. We finally sent him for CT scan. It is an abscess in an unusual place that can cause the same sort of problem. We will send him to the charity hospital tomorrow morning with the CT scan. Please pray that they will agree to do surgery to drain the abscess and clean it out.
We sent an adult last week for an abdominal abscess but not the same kind, and they did nothing for him. Thank you for praying.

Germil was picking cherries in a tree 2 days ago and fell. He was unconscious for quite awhile and then awoke and couldn't move. He showed up at clinic yesterday at closing. We put in a catheter but did not have the full story till today. On X-ray today we see he fractured 1 or 2 vertebrae in his neck. It is mango season and everyone is up in trees. We see many fractures and worse when they fall from a height. He is 60 years old. The poor especially are harvesting or just looking for dinner. We are attempting to find something to fix his head in position until the fractures heal solidly. We hope to prevent more loss of movement by keeping his head in a fixed position. The collar we have is not really sufficient. He currently has movement in his shoulders, arms, and less in his hands and nothing below his chest. Thank you for praying for him and fo us to help him.

Little Jonathan was delayed on his 2nd chemo. I think perhaps he was to have it today, but I have not yet heard. His type of lymphoma is very curable. Today a man returned after his mastectomy. We have had 2 males lately which is unusual. We had another patient have her surgery today as well. The surgery if appropriate is easier to accomplish at a nearly free Catholic hospital about 4 hours from here.

Chemotherapy is tough to accomplish here. For Jonathan, he and his parent must travel to Port au Prince which is somewhat dangerous and expensive. We will help out whenever needed. We do not always help financially, but we always offer gifts and prayers to help out. The lab work prior to chemo is free at our lab. We also give supplies that they would have to buy as a gift. Each person receives 1 or 2 liters of IV fluid, an IV tube, 2 sizes of catheters to insert in the vein, a 5 ml syringe and 3 kinds of meds to help with nausea and inflammation. The chemo itself costs about $100 which is very expensive for Haitians. The transport when going to Cayes costs about $40 round trip which is doubled as you need a helper to go along. I estimate that fewer than half of our cancer patients will go, due mainly to the cost. We will help those who have a good chance of cure, to go and will pay some or most of the fee for them. Most go every 3 weeks at least 4 times. Those with worse prognoses, go more times.

Your gifts to the poor fund help in so many ways. Thank you so much for your prayers for these patients and for the gifts to help them financially when needed.
