乳房大小與乳癌風險有關嗎? （2026-10-22）

48歲的Joelle（化名），育有兩名兒子。她過去大約每兩至三年會接受一次基本婦科檢查，包括子宮頸癌篩查及盆腔超聲波檢查，但卻從未進行乳房檢查。她認為自己胸部不大，患乳癌的機率應該很低，加上洗澡時偶爾自我觸摸檢查未曾發現異狀，因而忽略乳房篩查。 

近期，醫生提醒她已進入乳癌高風險年齡，建議她接受乳房X光檢查。結果，她發現一些異常鈣化點病灶。隨後進行切片病理檢驗，確診患上乳癌。所幸的是，腫瘤在第1期便被及早發現。 

不少人誤以為胸部大的女性，患上乳癌的風險較高，但此說法缺乏科學依據。大胸不代表患上乳癌風險高，小胸的也不代表安全。根據《香港乳癌資料庫第18號報告》，在受訪群組中，有近五成胸圍在 34 吋或以下；患者中約有 74% 為 B 罩杯或以下，只有小部分是D罩杯或以上。數字顯示並非乳房越大，患上乳癌風險越高。 

乳房大小通常主要取決於脂肪組織的比例，與乳腺多寡其實無直接關係，而乳癌則是發生在乳房纖維腺體組織(fibroglandular tissue)的疾病。所以，乳癌的真正關鍵是發生在乳腺組織中。乳腺與纖維組織比例較高，即乳房密度高，會讓乳癌風險稍微上升。同時，高密度乳腺在 乳房X 光檢查中會遮蔽微小腫塊，影響檢查準確度。中等緻密型乳房可能會掩蔽細小的硬塊，而極端緻密型乳房則會降低乳房X光的敏感度。因此，醫生一般建議在乳房 X 光造影外，加上乳房超聲波作輔助檢查。 

根據香港乳癌資料庫報告指，受訪患者中有70.7%屬中等緻密型乳房，8.2%則有極端緻密型乳房。緻密型乳房常見於年輕女性，但隨著年齡增長，乳房的緻密度隨之下降。絕經後，大多數女性的乳腺纖維組織由脂肪取代。 

若要說胸部較大的女性罹患乳癌風險較高，其背後真正原因往往與「肥胖」有關。因肥胖或過重導致體態豐腴，也可能增加患上乳癌風險。 

事實上，乳癌的風險受到多種因素影響，包括年齡、遺傳因素、家族病史、女性荷爾蒙水平、生活方式和其他環境因素。乳癌是一個非常複雜的疾病，無論乳房大小如何，女性都應該定期檢查。

乳癌篩查勝於事後治療， 存活率更高 （2026-10-20）

當一個人確診癌症，無論是病人還是醫生，最大的心願都是延續生命。以乳癌為例，隨著醫學技術進步，很多新研發的藥物能夠精準治療並延長壽命。然而，這些昂貴藥物所帶來的效益，仍不及定期乳房篩查及早診斷所能提升的存活率。

香港乳癌資料庫最新發表的專題簡報，估算乳癌的整體存活結果（由確診至因任何原因死亡）。透過篩查發現乳癌的患者，其10年整體存活率絕對增長（Absolute overall survival benefit）較因症狀出現才確診的患者高出 13.7%（88.0% 對 74.3%）。同時，其10年乳癌特定存活率亦較佳。這項優勢在第1期乳癌患者中尤為明顯，突顯定期篩查的重要。

對比乳癌新藥所帶來的存活率提升。以荷爾蒙受體陽性（HR+）乳癌為例，抗荷爾蒙藥物Tamoxifen 自八十年代起成為術後的輔助治療，患者15年的整體存活率絕對增長 9.2%。其後，醫學界在此基礎上加入標靶藥 Abemaciclib，其 6.3 年整體存活率再增加 1.8%。即使是較棘手的三陰性乳癌，患者在手術前後接受免疫治療Pembrolizumab，7年整體存活率亦提升 7.9%。這些數據顯示，昂貴的新藥所帶來的存活率改善，仍不及乳房篩查及早診斷所獲得的13.7%效益。

我們固然欣喜各類新藥物的面世，但若果婦女能透過乳癌篩查及早診斷並治療，10年整體存活率已較因症狀才確診的患者高。若再配合藥物治療所帶來存活率的提升，便能如積沙成塔般逐步累積，最終讓整體治療成果達至最佳。

近年香港的癌症防控策略，已由以往偏重「事後治療」，轉為「預防及早發現」。筆者期待乳房X光篩查能推廣至全港所有合適的婦女。正如常言所說：「預防勝於治療」，在疾病或問題出現之前採取預防措施，往往比事後治療或補救更有效和重要。

延續勇氣與愛 —「The Nathaliie Ng Days」全新療癒性帆船航行體驗】 🌊

香港乳癌基金會欣然宣布推出全新支援項目 —— 「The Nathaliie Ng Days – Therapeutic Sailing to Heal the Spirit Together」，透過免費療癒性帆船航行體驗，支援乳癌患者及康復者促進情緒復元、提升心理韌性、建立同路人連繫，並改善身心健康。

項目於10月17日發布，是已故藝人吳文忻女士的生日，以紀念她在對抗乳癌路上所展現的勇氣和對生命的熱愛。

此項目由香港乳癌基金會與專注共融及療癒帆船的非牟利慈善機構Sailability Hong Kong Ltd.攜手推動，並將成為乳癌支援中心的服務，旨在支援患者及康復者的身心靈復元、社群連繫及戶外體驗。

活動將於2026年11月至2027年4月展開，供免費參與。敬請密切留意，基金會將於稍後公布報名及活動詳情！

Honouring Courage & Hope | HKBCF Launches "The Nathaliie Ng Days" Therapeutic Sailing Program 🌊

The Hong Kong Breast Cancer Foundation (HKBCF) is pleased to announce the launch of “The Nathaliie Ng Days – Therapeutic Sailing to Heal the Spirit Together”, a new and free support initiative designed to promote emotional recovery, psychological resilience, peer connection, and psychophysical well-being among breast cancer patients and survivors.

The announcement is being made on 17 October, Nathaliie’s birthday, as a tribute to her life, courage, and enduring legacy.

Developed in collaboration with Sailability Hong Kong Ltd., a non-profit organisation specialising in inclusive and therapeutic sailing, the programme will become part of the services offered by the HKBCF Breast Cancer Support Centre to support survivorship, rehabilitation, and patient well-being.

The programme is free of charge and proposed for delivery from November 2026 to April 2027. Please stay tuned as the HKBCF will announce registration and program details in due course.

香港乳癌基金會（基金會）顧問理事會成員譚彩華女士（Wendy）於日前（10月6日）接受新城知訊台《樹窿78號》訪問。她在節目中介紹基金會乳癌支援中心的工作，並呼籲大家參加基金會的慈善步行籌款盛事 #乳健同行2026。

同場，支持基金會多年的群芳薈副主席馬振偉先生（Raymond）分享，過去連續六年參與 #乳健同行 ，活動已變成團體的年度約定。而這份支持更感染家人一同參與，今年團隊更突破參與人數！

During the programme, she introduced the work of the HKBCF Breast Cancer Support Centre and encouraged the public to join the HKBCF's signature charity event, .

Also appearing on the radio programme, Mr. Raymond Ma, Vice-Chairman of GF Federation, shared that his organisation has supported the HKBCF for many years. For the past six years, GF Federation has taken part in , making it an annual tradition for the group. Their support has also inspired family members to join in, and this year the team achieved a record number of participants!
