Last week, one of the biggest conversations in prostate cancer treatment changed.
For years, one of the main reasons given for not routinely discussing focal therapy was the lack of long-term evidence.
Today, that evidence exists.
Now the big question is whether every man will hear about it.
A landmark UK study following 3,477 men over 10 years has demonstrated excellent long-term cancer control for appropriately selected men, with survival outcomes comparable to radical treatment, but with significantly fewer life-changing side effects.
This doesn't mean focal therapy is right for everyone.
It does mean every suitable man deserves to know it exists and to have the opportunity to discuss whether it could be appropriate for them.
The science has moved on. Now the conversations need to catch up.
Because one of the biggest decisions of a man's life should never depend on where he lives, which hospital he attends, or which consultant he happens to see.

For decades, breast cancer treatment has evolved to preserve healthy tissue whenever it is safe and appropriate. Many women are now offered breast-conserving surgery instead of mastectomy.
