Background
Survival from out-of-hospital ventricular fibrillation cardiac arrest is extremely variable throughout Europe with high survival rates of more than 50% and low survival rates with single digit percentages. Even more fundamental is the fact that only very few communities in Europe have rigorously measured and reported their survival rates. Most communities have n

o idea how they are performing in terms of managing sudden cardiac arrest. As proven in several communities within Europe, like Stavanger/Norway, Canton Ticino/Switzerland, Helsinki/Finland, etc. managers and directors of EMS systems can learn the steps they need to take to improve survival. The Mission
The goal of the European Resuscitation Academy is to improve survival from cardiac arrest. This will be achieved through the enrollment of EMS managers, EMS directors and EMS medical directors, in a program conducted by the European Resuscitation Academy and the local host community. Specific Goals:
- Learn how to define the cardiac arrest survival rate in the host community.
- Understand the principles of the Utstein template and how to report data.
- Develop a concrete plan of action to improve survival.
- Measure the effect of the plan of action on cardiac arrest survival.

"Investigators reviewed 29 available videos from 1990-2017...the rescue process of each collapsed player was analyzed with careful attention paid to the first action performed by the first to arrive on the scene...65% showed actions to prevent tongue swallowing, which included placing the player on his side or tilting his head sideways, and forcefully opening the athlete's mouth placing the rescuer's fingers in the victim's mouth, sometimes with a visible pull at the tongue. Only 38% show chest compressions. Further, a defibrillator was brought to the scene in only two cases, and in one of those, the first shock was not delivered until 10 minutes later. Of the players presenting with cardiac arrest, 36% survived."

"The health and economic burden of SCAs is high, predominantly underpinned by very high mortality. Annual national losses approach AUD$2 billion (USD$1.42 billion) and are comparable to productivity losses from all cancers combined. Prioritising research and state-of-the-art care for SCA patients appears economically sound."

There are 20,000 sudden cardiac arrests (SCAs) in Australia annually, with 90% case-fatality.

Conclusions
During the COVID-19 pandemic in Switzerland mortality increased in Cantons with high-incidence of infection, whilst not in the low-incidence ones. OHCA occurrence followed an opposite trend showing how variables related to the health-system and EMS organization deeply influence OHCA occurrence during a pandemic.
