The Directors of the Hants Shore Health Association (HSHA) are pleased to announce that, effective May 25, 2026, the services of the family practice of the Hants Shore Community Health Centre (HSCHC) will transition to a Nova Scotia Health operated health home.
This transition has been under discussion for several years and received strong support at the Association’s Annual General Meeting last January. Importantly, patients can expect a smooth transition. While there may be new signage and minor administrative changes, patients will continue to be welcomed and supported by a dedicated reception and clinical team, including many familiar staff members they know and trust.
Adapting to changing circumstances and exploring new service models is essential to maintaining a strong health care team in our community. This new arrangement is designed to support the long-term stability of local health services while strengthening our ability to attract and retain qualified health care professionals.
The community will continue to own the building and remain responsible for its upkeep. By transitioning to a landlord role, the HSHA will help ensure continuity of care and create a sustainable foundation for the future delivery of health services in our community.
The health home has a patient intake process in place. It is not a walk-in service and is not accepting new patients at this time. The team is working through onboarding existing patients of the clinic, and will then be accepting patients from the Need a Family Practice Registry. To register for the Need a Family Practice Registry, or to update your contact information, update your health information, or confirm your registration, please visit needafamilypractice.nshealth.ca
If you are a patient of the Health Centre and have questions about this transition, please contact the office directly and our team will be happy to assist.
