Peter had always dreamed of going on safari in Kenya.

When his health meant he could no longer make the journey, Hammond Foundation supporters helped bring a little piece of that dream to life through the Dreams Program.

Peter enjoyed a special safari experience at Taronga Zoo, where he fed giraffes, met incredible wildlife and stayed overnight at Taronga Wildlife Retreat.

One of the highlights was calling his daughter, who was on safari in Kenya herself. As Peter fed giraffes in Sydney, she stood among zebras on the Kenyan plains, sharing stories about the animals they had seen.

For Peter and his family this experience is a treasured memory they created together. As his wife Gina later reflected, Peter "never stopped talking about his Taronga Zoo experience" until he peacefully passed away.

We estimate it costs almost $18,000 a year on top of Government funding to care for a resident living with complex dementia.

We all agree that every person living with complex dementia deserves the right care, in the right place, at the right time – but Australia’s aged care funding model is leaving too many people stranded between hospital and the specialised care they need.

On this World Alzheimer’s Day, we’re advocating for new pathways to meet that gap, so that no one is left without the care they deserve.

A dream doesn't have to be grand to make a difference.

For Chizuru Sammut, it was a peaceful sensory walk in nature inspired by the Japanese practice of forest bathing. For her family, it became a treasured memory.

Through the Hammond Foundation's Dreams Program, Chizuru's dream became a reality, creating precious moments with family and lasting memories during a difficult time.

Last week, we celebrated the 15 year anniversary of the Hammond Foundation.

Since 2011, our generous community of donors, partners and advocates has raised more than $46 million, improving the lives of over 20,000 people and families.

From fulfilling end-of-life dreams and providing practical support for vulnerable people, to helping build HammondCare Darlinghurst and investing in the future of dementia and palliative care, every contribution has helped create lasting impact.

We reflected on the difference this support has made as well as the opportunities ahead. As Australia's care needs continue to grow, so does the opportunity to shape the future of care.

Caring when others can't or won't: HammondCare team members Dan and Kacey saw a desperate need, and acted.

When Dan Mousa arrived for a routine home care visit in Newcastle, he found an older man frightened, alone and living in unsafe conditions.

Dan reassured him and, together with Care Operations Manager Kacey Snell, began contacting services and agencies for urgent assistance. Despite the obvious need, no immediate help was available.

So they acted.

Thanks to financial support from the HammondCare Foundation's Care Fund, Dan and Kacey arranged emergency accommodation, purchased food and essential supplies, and made sure he was safe for the night. They continued advocating for his ongoing care and wellbeing.

Their actions went far beyond what was expected, demonstrating the compassion, determination and willingness to act when others can't or won't that sit at the heart of HammondCare's mission.

Dan has been recognised with a HammondCare Mission in Action Award.
