Last week, we celebrated the 15 year anniversary of the Hammond Foundation.

Since 2011, our generous community of donors, partners and advocates has raised more than $46 million, improving the lives of over 20,000 people and families.

From fulfilling end-of-life dreams and providing practical support for vulnerable people, to helping build HammondCare Darlinghurst and investing in the future of dementia and palliative care, every contribution has helped create lasting impact.

We reflected on the difference this support has made as well as the opportunities ahead. As Australia's care needs continue to grow, so does the opportunity to shape the future of care.

Caring when others can't or won't: HammondCare team members Dan and Kacey saw a desperate need, and acted.

When Dan Mousa arrived for a routine home care visit in Newcastle, he found an older man frightened, alone and living in unsafe conditions.

Dan reassured him and, together with Care Operations Manager Kacey Snell, began contacting services and agencies for urgent assistance. Despite the obvious need, no immediate help was available.

So they acted.

Thanks to financial support from the HammondCare Foundation's Care Fund, Dan and Kacey arranged emergency accommodation, purchased food and essential supplies, and made sure he was safe for the night. They continued advocating for his ongoing care and wellbeing.

Their actions went far beyond what was expected, demonstrating the compassion, determination and willingness to act when others can't or won't that sit at the heart of HammondCare's mission.

Dan has been recognised with a HammondCare Mission in Action Award.

After a life-changing stroke, Sonja's father Rajko (pictured with his family) arrived at Braeside facing major challenges with movement, speech and independence. What made the biggest difference was not only expert rehabilitation, but the positivity, kindness and encouragement of the whole team - helping him rebuild confidence, continue therapy, return home and even get back to work.

The team reassured him during moments of frustration and helped his family feel confident he was safe, understood and genuinely cared for. They provided practical support at every step, from physiotherapy, speech pathology and occupational therapy to helping secure NDIS funding so his recovery could continue beyond hospital.

“When nothing else was certain, it was clear and obvious that he was being taken care of. We are eternally grateful for the staff and their care.” said Sonja.

This National Stroke Week, Sonja’s story is a powerful reminder that recovery is about more than treatment. It is also about compassionate care, emotional support and the practical help that gives patients and families confidence for life after stroke.

A morning at South Sydney Rabbitohs HQ brought back wonderful memories for HammondCare Darlinghurst resident Edward, pictured right, alongside resident Sella and Rabbitohs halfback Jamie Humphries.

Growing up in the area, Edward’s dad used to take him along to the games when he was little.

HammondCare Team member Mark says, ‘these outings are more just than a day out enjoying the sunshine. They also create opportunities for reminiscence and reconnection. We’re so grateful to the Rabbitohs for their generosity and hospitality.’

This is Ted (left), a regular visitor at HammondCare’s Forest Oak Social Club, and his new friend Daniel, who also attends the club.

Ted enjoyed a fulfilling career as an architect and professional calligrapher, then his life took an unexpected turn after he retired – suffering from a stroke while on an overseas trip.

Ted has since taught himself to write again. The care and community he experiences at the Social Club has brought a renewed purpose back into his life, where he can step out of his usual routine, share a fresh cooked meal, and connect with others in his local community. 'For that,' he adds, ‘I am deeply grateful.’

His family also have the peace of mind knowing Ted all the support he needs while enjoying the activities on offer..

Meet Herbert, a resident at HammondCare Darlinghurst, our first care home for people at risk of homelessness. He’s joining six other residents, staff, and vital volunteers, for a special concert at the Sydney Opera House.

As part of the Sydney Symphony Orchestra's Music for Health program, Herbert and friends were invited to watch violin virtuoso Gil Shaham playing Beethoven’s Violin Concerto with the orchestra. The SSO also generously provided accessible transport to and from the event.

‘We’re so grateful for this partnership,’ shares HammondCare team member Kate. ‘The SSO gave a chamber concert at HammondCare Darlinghurst earlier this year as well – creating more joyous moments for residents, especially those unable to travel.’

A generous donation from a resident at HammondCare Caulfield has started a wonderful new chapter for the local community.

Michael, an avid book collector, has donated more than 700 beautifully bound classic books to the care home.

Growing up with just three books – his mother’s cookbook, a dictionary, and his father’s book on aviation – Michael began building his own immaculate collection once he started work at 17, often waiting until payday to buy the next instalment.

Now his treasured books have found a permanent home at HammondCare.

‘They’ve given me a lot of pleasure in life, and now they’ve come along with me to my new home,’ Michael shared at the ribbon cutting ceremony. Thank you Michael!
