Today one of our Support Coordinators attended a seminar hosted by Legal Services Commission South Australia.

What is an Advanced Care Directive (ADC)?
An advanced care directive is a legal form/document completed by an individual over the age of 18 to write down their wishes, preferences and instructions for future health care, living arrangements, personal matters and end of life. This document also has the individual appoint one or more substitute decision makers
