PROGRAMS @ ECoTAC

The Tasmanian Home and Community Care (Tas HACC) program funds community services for Tasmanian's under 65 years of age or under 50 for Aboriginal people, and their carers to live independently at home if their capacity for independent living is at risk due to:

- a health event
- living with an ongoing health condition people that need ongoing support and do not meet the eligibility for the NDIS.

The service aims to help people stay well in their own homes.

The Elders Council facilitate a HACC program and have done so for many years. As a service we can provide Individual Support + Advocacy, participation in Social Support Group activities, Meals and Transport where available.

The HACC program is a self referral program via the Tasmanian Community Care Referral Service. Just follow the attached link to gain further information on the HACC program or to access the self referral form. Acceptance into the program is circumstance based and will be assessed on a case by case basis.
